New Patients » Gessler Clinic

We welcome new patients for consultations with our physicians, diagnostic evaluation and medical treatment. You may contact us directly or through a referring physician. Prior to your first appointment, please forward copies of your medical records from other physicians. You may download, print and complete the medical questionnaire (printable PDF file, 108 kb, opens in a new browser window) and bring the completed form with you to your appointment. We ask that you arrive 30 minutes early to allow for parking and timely arrival.

For your first appointment, please bring:

• Your medical records (or make sure that we have received them).
• A list of all current medications including herbal and over-the-counter medicines, and nutritional supplements.
• Your health insurance card(s). You will also be asked to present your insurance card(s) on all subsequent appointments. If co-payments are required by your insurance, they must be made prior to seeing the doctor.
• Photo I.D.
• If patient is a minor child and not accompanied by custodial parent, then a notarized statement for authorization of treatment must be brought in by the person with the child.

Appointments
We encourage you to make your appointments as far in advance as possible. We recognize the importance of your time and will make every effort to meet your scheduled appointment. We ask your understanding when medical emergencies may occasionally cause us to delay or reschedule your appointment. Please notify us as soon as possible if you must cancel your appointment, or be unavoidably late. If you must cancel your appointment due to last-minute unforeseen circumstances, please let us know immediately. Effective January 1, 2009, Gessler Clinic, P.A. charges existing patients $30.00, and new patients $50.00, for any appointments not canceled at least 24 hours in advance.

Insurance and Billing
Gessler Clinic will assist you by filing your claim with Medicare or your private insurance company. If you have secondary insurance, we will file this for you as well. Contact your insurance company prior to scheduling an appointment to confirm provider participation with your plan. If you have questions regarding insurance or billing, or which insurance providers are accepted by Gessler Clinic, you may call the Insurance and Billing Department at 863-298-3355.

Parking
Patient parking for Gessler Clinic main office is located on the northwest side of the building, with additional overflow parking available to the west of the main parking lot. For our patients’ convenience, there is also a covered patient drop-off area at the main entrance. Parking for our other divisions is located adjacent to the entrance of each building.
